can anyone tell me what character appearance,
Describe what your character looks like. There is a table in the player handbook to randomly determine his heigth and weigth, you can use it if you want. Or you can simply describe him as you see him (he's short and fat, he's of average heigth and very muscular, he's very tall and thin). Add the skin, hair, eyes color. Does he have any scar or other distinctive trait?
You could try to include a picture of your character. Check a site like Elfwood (http://elfwood.lysator.liu.se/elfwood.pike?1029) to search pictures of fantasy character. The Wizard of the Coast site have also a few black and white portraits. I think there is a forum here for character portraits too...

traits, disadvantages,

Traits describe your character's personality. Is he curious, party animal, cautious, talkative? A disadvantage could be something that your character is especially bad at, making him unique. Perhaps is he affraid of water, fire or the dark? It could simply be a skill you take no rank in.

Check in my sig the 1001 Character Quirks for more traits and disadvantages.

The book Unerthead Arcana has the Traits and Flaws concept, I don't know if you use it in your campaign (ask your DM). A trait something that gives you a little bonus to a skill and a malus to another skill. A Flaw is the reverse of a Feat, it's a disadvantage; but taking a Flaw allow you to take a bonus Feat. See here (http://d20srd.org/indexes/variantBuildingCharacters.htm) for more information on this section.

...history, goals in life are and what i should put?
History and background: Who are your parents and siblings? What did you do before begining adventuring? It's helpful for your DM if you put some plot hooks. For example, if you say your sister was forced to marry an ugly noble and the DM wants to design a plot in nobility, you could say your character recieved a letter from your sister asking you to visit her. Once again, check the various links in my signature for suggestions.

Goals in life: If you want your character to take a Prestige Class, it could be a goal in life, or at least a mean to achieve your goal in life. He could wants to rule a dukedom, avenge his parents killed by [INSERT CREATURE/EVIL RACE HERE], make money, spread his ideal or his religion.
