Allowing a 20 (nice even number). 6 level fighter (dwarf)
So thats 6 BA + 5 STR Total 11

1Power Attack
FB Improved Sunder
FB Improved Bull Rush
3 Combat Brutemomentum swing (Complete Warrior)
FB Shock Trooper headless charge (complete Warrior)
6 Heroic Surge (SWRPG p110)
FB Cleave

Heroic Surge You may make an extra Move action or attack action, This action may come before or after you regular actions.

Start by charging and taking a full power attack
assuming a mundane dwarvwn War axe your now attacking at +8 and getting and doing 1d10 + 10 (-2 AC)

Now use headless charge so take your -5 on your attack role and apply it to your AC instead of your attack roll. so now we look like +13 doing 1 d10 +10 (-7 AC)

Now use your extra attack action to attack again still charging this round use Momentume swing. So now your doing an 1d10 + 25 and your attacking at a +13 at sixth level. All for a net of -7 AC on the AoO.

The orginal idea is that these dwarves would charge the enemy in large swarms, so you being hit my multiple guys in one round and you can't AoO all of them.
