The thread title says it all. After some consideration, I thought perhaps Nerull's followers would be those who lurked in the shadows and struck out at whomever to make sacrifices to him. However, I'm not sure that this is remotely accurate--can anyone help me out?

Complete Divine has something to say about this. Without going into enough detail to get into CoC violation, Nerullites are not subtle. These are the guys founding cults for vampire food, and raising the big undead armies, etc, the showy death psychopath stuff. They keep their bases of operation secret but you will likely see one and his undead homies coming a mile away when he goes to work.

Ahahaha! That's awesome, but it did kind of kill my idea of what a priest of Nerull was like. What does CDiv have to say about servants of Hextor?

Military focused tyrants obsessed with having "lessers" beneath them.

I played a cleric of Nerull in our last campaign. My group agreed it was the creepiest character any of us have ever played. I played a lot to the death is the purpose of life. Pretty much he treated everything except clerics of Nerull as sheep that had to be shepharded to Nerull's flock (by ending their lives of course).

The best part was when we found a badly injured NPC (who was going to be critical to the DM's story) and instead of healing him, my character death knelled him to send another soul to Nerull's realm.
