I'm kind of leaning towards a bare knuckle fighter for my next character and I'm interested in what combinations of races, classes and feats the community considers best for this. While a monk is the obvious choice I am curious as to the possibilty of a barbarian and I am not so well versed in D&D that I can discount the existence of other classes or prestige classes that might prove useful. I would like to stick relatively close to humans but anything with up to a +3 LA might be acceptable.

Insectile Human (Savage Species, LA +2) Monk6/Shintao Monk6 (Oriental Adventures)/Barbarian or Fighter6.

Take the MultiWeapon Fighting Chain and grab a Monk's belt and you will be laying down the pain.

You should look into the Shou Disciple PrC from Unapproachable East. In 5 easy levels that wonderful little PrC grants 2d6 unarmed damage, two bonus feats, an AC bonus that increases the effectiveness of the Dodge feat, Full BAB, and two good saves. It also grants the ability to use any melee weapon in a Flurry of Blows attack, but you need to have a level of Monk (or perhaps another PrC) that grants the Flurry of Blows ability for that to become useful.

Also note that other than increasing your unarmed damage from 1d10 to 2d6, the 5th level of Shou Disciple doesn't do you a lot of good unless you have Flurry of Blows.

If you wanted to stick with a straight Barbarian up until entering Shou Disciple, you'd have to go to Barb 6; but you could throw in a level of Ranger, Monk, Bard, or Rogue to meet the requisite Reflex save earlier.

You could go Human Ranger 1/Barbarian 4/Shou Disciple 4. I'd allot your feats thusly:
H-Dodge
1-Improved Unarmed Strike
3-Weapon Focus (Unarmed)
6-Snow Tiger Berserker (Unapproachable East)
7-Power Attack (bonus feat)
9-Cleave?, another bonus feat

After that (or before) take another level of Ranger to pick up TWF if you want.

Well if you wanted to do some kind of monk barbarian levels, you'd have to start off with monk first, then move into barbarian levels. You keep your monk abilities if your not lawful, but you loose your barbarian abilities if your not chaotic. >.>;

I would take your meaning to be 'Make an unarmed guy who isn't a monk."

I would suggest focusing on screwing with people. CW has some good stuff.
