Cleric 5/Exorcist of the Silver Flame 10/Mindspy 5.

This gives you a detect thoughts DC of 20 + Cha mod, which is much higher than any of the other classes or abilities that I have seen. Plus, you get 6th level divine spells, and since you're raising your Cha, you can easily get enough turn undead uses for lots of divine metamagic.

So, any thoughts?

Ya, Succubus/ Mindspy/ Mortalhunter does it better, with a DC of 10+half HD + Succubus Charisma, plus they have it unlimited times per day, can use Supernatural Transformation, and Ability Focus as well.

If you want to play a good Mindspy, try to convince your DM to play a "gifted" human who has Detect Thoughts as a (Sp) and some bonus Charisma, at the cost of +1LA.

There is also a great feat from LoM where if you have the telepathy ability, you automatically pinpoint the location of every sentient being in your radius. This also lets you "see" a bunch of characteristics about said characters. I think the feat is called mindsight, but I don't have my books with me here. The feat is buried in one of the creature descriptions.

Yep, the feat is Mindsight, it's buried in the Tsochai discription, near the end of it, a useful feat.

My preferential Mindspy feat (EDIT: ahem...build) is Eberron based (it's also a Cabinet Trickster build, but they're both 5 level classes, so...)

Hexblade 4/Warshaper 4/Mindbender 2/Cabinet Trickster 5/Mindspy 5

You get the all-important +16 BAB for epic, Detect Thoughts at will, some useful Mind Tricks, and Telepathy (thus the Mindsight feat), and the feats are pretty much open. Not to mention at +4 to Strength and Con and an immunity to Crits and Stunning, as well as any natural weapons you can dream of.

I simply love the Cabinet Trickster 5/Mindspy 5 (the fifth level of Mindspy is unnecessary if you're tight for build space, but it meshes so well with everything)

The Mindbender PrC and Mindsight feat are must-haves. Hexblade4 or Warlock6 are the minimum levels required for it. A lenient DM might also allow the half-vampire template (Libris Mortis) to qualify since it gives you charm monster at will (a stronger version of charm person) at a caster level equal to your HD. Also, the template doesn't hurt your CT thought trick ability since it's based on your character level and not hit dice.

How's this for something weird:
Half-Vampire 2/Paladin 3/Rogue 2/Mindbender 1/Pious Templar 1/X 1/Cabinet Trickster 5/Mindspy 5
- Charisma to your saves, evasion and mettle, instant detect thoughts at will, telepathy, thought tricks, charm at will, etc.
- The rogue levels are there just for evasion. Monk works too, or ninja if you want invisibility.
- The free level can be for anything: 2nd level of mindbener, Warshaper (always good, and gives you the disguise skill)
- the last level of Mindspy can be dropped if you want to squeeze in anything
- if you're not too strict with alignment constrictions (or play alternate Paladins), you could even fit in Warlock 2 for detect magic

edit: after reading another thread, the fey-touched or half-fey templates probably work better

I kind of thought a Half-Illithid Mindspy would be cool, if completely unoptimized(3 uses of detect thoughts a day wooooo!). A Helm of telepathy fixes the problem. Although I believe the save on a helm of telepathy is very low.

One question: Mindsight says that you know exactly what square someone is in as long as they are in your range of telepathy, but doesn't negate concealment. True Strike gives +20 and lets you ignore concealment miss chance. Is this a free hit against someone invisable or am I reading something wrong.
