First you have to avoid damage from an AoO you provoke when attempting to start a grapple. (not considering improved grapple)

Second you have to succeed on a melee touch attack.

Third you have to succeed on an opposed grapple check.

If this is successful then you are considered to be in a grapple and can try a number of things like pin etc... Can you do this in the same round you started the grapple or do you have to wait till the next round?

So can you pin someone in one round, or does it take 2 rounds i.e. one to grapple, one to pin...?

Any and all of the grapple checks are made in place of any of your normal attacks. So if you have at least 2 attacks in a round from BAB(I'd allow the monks flurry-but that's debatable), in theory, you can grapple and pin in the same round. This is of course, made at the normal 5pt lower BAB as you continue to make the checks making pinning in the same round less likely, but still possible.These attack forms substitute for a melee attack, not an action. As melee attacks, they can be used once in an attack, or charge atcion, one or more times in a full attack action, or even as an attack of opportunity.
