If a cleric has cast sanctuary while a barbarian is grappling her, what happens? Can the barbarian continue to grapple her, even attempting to pin her, or does the barbarian let go against his will?

By my reading of the spell, I see nothing that would make the Barbarian drop the cleric, but he would also not be able to pin the cleric (as that takes an attack action to make the grapple check.)

I personally would rule that that the barbarian is already engaged with the cleric and could not be affected by the spell (using common sense rather than the wording of the spell.) I do realize that this would be a house ruling.

You can attempt to cast a spell while grappling or even while pinned (see below), provided its casting time is no more than 1 standard action, it has no somatic component, and you have in hand any material components or focuses you might need.

Since Sanctuary has a somatic component, it can't be cast while the Cleric is being grappled.

Neither of the above. The barbarian simply cannot make attacks against the cleric, including grapple checks made in lieu of attacks.

The spell describes striking or otherwise directly attacking (including with a targeted spell) as being forbidden after failing a will save. My interpretation is that this is different from "using an attack action".

Any action you could do that doesn't involve trying to do immediate direct harm (including situational harm, whether or not it does actual damage) to the warded opponent would be okay, including making a grapple check to draw a light weapon or escaping from a grapple. So, there is no impasse triggered by this situation that forces the barbarian to continue grappling until the spell ends.

A bit more nebulous is whether actions like helping someone else escape a grapple would be permitted or not. I would lean toward permitting such actions, since any harm caused to the warded character would be indirect.
