Will a Headband of Intellect give you additional skill points at each level? If so, how do you deduct them if the item is lost or destroyed?

No. In fact, it's explicitly stated that it does not grant you skill points when you gain a level.

No items affect skill points earned at level up unless they change your permanent intelligence score. Which as far as I know is limited to the Tome of Clear Thought. There might be other items or magical areas that affect your intelligence score, but that's the main way.

How about other level-up activities, like selecting feats?

For example, <i>Combat Expertise</i> requires Int 13 -- would a character whose Int is 11 naturally be able to qualify for that feat when wearing a Headband of Intellect +2?

What I'm too lazy to dig it up but from what I've heard you can take the feat but if you ever lose the item or enter an Anti Magic Field you lose access to the feat and any feats that depend on it.

When an ability changes, everything that depends on that ability changes unless the source of the change explicitly excludes the change from affecting the stat.

For skillpoints, the Headband of Intellect explicitly states no extra skill points are gained. This is reinforced by the rules on levelling up in the PHB which state (among other things) that a character with ability damage still gets his full regular allotment of skillpoints. (That seems silly to me but it is what the PHB actually says).

Enhanced abilities do qualify the character for feats that the increased ability would grant. If your ability drops (for any reason, not just losing the item) you can no longer use the feat. You still have the feat (if it matters for anything else) you just can't use it.
