One of my characters recently lost an eye. Are there any official rules on the nature of any penalties that might be associated with this? And if there isn't, do you think a -2 to Spot might be a enough? It was a character choice, so I don't want to punish him too much.

I wouldn't necessarily say there is any penalty. A minus 2 to spot could work but in many ways having one eye instead of two doesn't make you less aware. If anything, the negatives to spot would only happen if someone approached from the side where he was missing the eye. Then again...his head can turn.

I would, perhaps, give him penalties to attack and skill checks for a little while but after a given time have all penalties go away. This way it represents him 'adapting' to having one eye.

DMG page 27. There's a reason we have two eyes. Depth perception makes everything a lot easier.

Bah, the DMG is overly punitive with those penalties. Depth perception based on binocular vision is only good out to a few dozen feet anyway.

-2 Spot and Search is probably sufficient, especially if the character has had time to adapt to using only 1 eye.
