Can bullrush ever cause damage? For example, let's say a PC bullrushed a monster into a wall. Would the monster take damage?

You can bullrush a NPC into a terrain that deals damage. Bullrush by itself causes none.

Warforged Juggernauts (Eberron Campaign Setting) gain an ability to deal damage when they Bull Rush.

In Dungeonscape, there's a fighter alternate class feature called Dungeon Crasher that you can take in place of your 2nd level feat. And, among other things, it does just what you describe: dealing damage to an opponent when you bullrush him into a door, wall, statue, or any other solid object.

You can use the (rather amusing, IMHO) Driving Attack feat from PHBII.
