As far as I know, yes because it reflects that you're smaller and harder to hit.

Imagine the inverse, if the penatly for large sizes didn't a ngore would get easier to hit if caught flat footed. And if that example doesn't work out right that is cos of the stats of an ogre, don't pick at that one thing.

Remember, you lose any bonuses to AC (of specific types in the case of flatfooted) but not penalties. It's the same as how someone with a DEX lower than 10 doesn't become harder to hit when they're flatfooted.

Yes, the size modifier to AC is always present. This is why it's harder to hit a 1-inch target @ 30 feet than it is to hit a 50-ft target @ 30 ft. The size is the difference.
