This post is completely random, but I had to write a character analysis for a monologue for my Drama final tomorrow, and I realized how much a simple thing like this could add to roleplaying.
Do you think it is a little out-of-line to force your characters to fill a simple questionaire?(I usually require a basic 1-2 sentence physical description, a 1-3 sentence basic outline of mental triats, and a very basic background from my players). Also, what are your favorite questionaires(I saved a 100+ question one that someone posted a while back, but that is a little crazy)? if you want I will post the relevant questions from the analysis sheet that I had to do.

P.S. Sorry about the poor editing, and random nature of this post, I am fairly tired and it was an arbitrary thought.

The character development series of questions in the Hero Builder's Guidebook is pretty good for this, and is what I use, even when the players have already finished building their characters. Instead of rolling randomly, the "already done" PCs fill it out so I have a lot of backstory details.

Most importantly, questionaires should be short and too the point. The goal of a questionaire is to help the (1) player and (2) DM get into the mind of the character a little better. The longer it is, the less likely it is to be completed and the more likely the player is to get bored.

In my campaign, I "bribe" my players with token XP (roughly equivalent to 1-2 encounters per level) for metagame contributions that advance the story and characters. This includes a PC questionaire and in-character write-ups.

The questionaire can be found at http://users.bigpond.net.au/nom/d20/guide/quiz.html .

Check the links in my sig. The Ten Tips are quick and to the point, it could be a good start.
