Damage during casting requires a concentration check for each individual attack. I'm not sure where your "combined damage" bit comes from since it doesn't appear anywhere in the rules. :rolleyes: |

Correct. Only damage taken at the exact moment of casting requires a Concentration check. |

There is an exception. damage from a continous source is considered to have half come at the time of casting. So Melf's Acid Arrow can force its concentration checks even if not cast at the same time as the spell it's disrupting. |
