Can a Changeling always identify another Changeling?

Nope, although the +2 Sense Motive might help a little, for some interactions.

Also, small groups of changelings might develop tells or signals for each other, but that wouldn't really be shared outside their group.

Although it does make one wonder how the race survives generation after generation :)By breeding with other species. If memory serves, when a changling and a non-changeling breed, the offspring is a changeling if its the same gender as the changeling parent.

Plus, it also has to be said that not all changelings make a secret of being a changeling.

That's kalashtar. Half-changelings simply have a 50% chance of being changeling, regardless of gender.
