It was recently produced by a D20 publisher that is currently out there. I went to trolllord.com to find it. It gives the history of names here on earth and divides all the names by country, time era, and gender. Toward the end of the book, it tells how to come up with fantasy names as well as names for inns, taverns, buildings, and the like. I wish WoTC would also produce such a book.
