Hey guys, I'm taking and modifying the plot hook from Five Nations with the Blood of Vol summoning a creature -- The only thing is, I can't decide what .
I wanted something to be in the range of CR 13-15, but if it escapes being destroyed by the PCs after its immediate return (ie, still "weak"), it'd be advanced and made significantly stronger.
I even toyed with it it being a Rajah, most unexpectedly by the BoV, or a Phantasmal Killer (HoH).

I prefer the Terrasque...

I do believe that the Tarrasque is quite a bit above CR 15.

If you're using HoH, what about an elder taint elemental? It would fit with the context of the lake, and the Khyber shards could be intended for an attempt to bind the thing, but of course something goes horribly wrong and it gets loose. As it instinctively makes it way to the Madstone, the PC's have a chance to stop it. But if it reaches the Madstone, it will leech the souls trapped within and grow even more powerful.

Indeed it is (CR 20), but that doesn't mean it can't be modified to be a lower CR.

I find the Tarrasque incredibly dull and passe. Elemental is a good idea.

Any more suggestions?

Not exactly anything helpful for you, but I've toyed with that as a way to introduce Evangelions into Eberron.

More on topic, a black, green, or white dragon--perhaps with an interesting template--could fit the bill. I'd call the creature an experiment of the ancient dragons, which they sealed in the lake to prevent it from ravaging Argonessen. A twelve-headed pyro or cryohydra could also fit the bill.

If you're willing to use psionics, an aquatic version of the neothelid might also make an interesting encounter, though how you would explain its existence is beyond me.

Sounds like you should make a custom entity something like Molasar from The Keep (http://www.amazon.com/gp/product/0812579259/002-6606953-6793653?v=glance&n=283155).

Also made into a movie (http://www.imdb.com/title/tt0085780/) by Michael Mann.

Molasar is pretty much a vampiric entity.

He first appears as a skeletal being wrapped in a swirling fog. His abilities are pretty weak, but he is still a supernatural being after all.

As he kills mortals and absorbs their lifeforce, Molasar becomes more coporeal...

and big...

This Website (http://www.the-keep.ath.cx/default_en.htm) has a lot of info on Molasar.

It even has scans of the AD&D module that Role Aids (always loved the name of that company) adapted from the movie script

If you've got HoH, then homebrewing an entity like Molasar should be easy.

Euangelion is on the right track.

You could use a Nightwalker out of the MM...
