I was just wondering about the perfect two weapon fighting feat which states that you can make as many attacks in your offhand as you can with your primary hand. Does this mean that if you get an extra attack such as from haste or the speed weapon property and have 5 attacks in your primary hand you can make 5 attacks with your offhand also?

If both your weapons are speeded, yes. Otherwise, no. At least in my games.

I think this is one of those vague areas in the rules that has never been clarified either way. Myself, I don't use PTWF because there's really no need for the fourth TWF feat to be epic. I use superior TWF which is available with 21 Dex and +16 BAB; the wording is exactly the same as the other TWF feats.

If using it exactly as written you get an offhand attack for every attack you get with your primary hand no matter how you get the primary attack.

For example a Fighter 10/Dervish 10 that with a Speed weapon. He gets 4 primary attacks for BAB, which can be doubled 1/day via the Dervish ability Thousand Cuts, plus one for the Speed weapon for a total of 9 attacks with his primary hand. Perfect Two Weapon Fighting as written would then give him 9 attacks with his offhand as well.

You know, as much as I don't think that was the intention, being an epic feat I almost think that it should work that way. Because if it just gives you 1 more attack than Superior TWF, and at -15 to hit, that doesn't seem all that epic to me. I am not sure what the intention was with that feat. They certainly didn't know about Dervish abilities when it was written. I can't imagine it should give you an extra attack with a non-speed weapon, but if you are hasted, you should probably get 2 extra attacks.

Another thing it should have done was allowed you to get 2 attacks as part of a single attack action. That would be the real benefit for a lot of high dex fighters - they could avoid trading full-attacks with Spring Attack and this feat, and make some inroads against high-STR fighters.

Well it was also found in Complete Warrior with the Dervish PrC.
