Make an opposed grapple check as a free action. If you succeed, you and your target are now grappling, and you deal damage to the target as if with an unarmed strike. What book is Great Throw from? Defensive Throw does something similar to what you're describing, but it's a simple trip attack, the opponent isn't thrown to a different square.
