I'm working on a build for a friend who just wants to attack like crazy. He'll "worry about actually hitting later".
So, I'm looking for classes and feats that grant additional attacks.
The material should be 3.5, but 3.0 is concidered.
While I'm looking at this build at up to 6th level, I'd still love to see anyone's higher level options.

CLASSES:

Monk: Flurry of Blows

Chaos Monk (Dragon Mag; Monk variant): Flailing Strike - When making a Full Round Attack with either Unarmed Strike or a Kata (i.e., special monk) weapon, the Chaos Monk has the option of taking the listed Penalty (initially –2, reduced to –1 at 5th level, and becoming –0 at 9th level) to receive a random number of extra attacks (minimum +0), beginning with a roll of 1d4 -1 and going up to 1d6.

Kensai (Dragon Mag; Fighter variant): 2nd level option of a feat or "Rain of Blows" ability - As part of a Full Round Attack, the Kensai may take a –3 penalty to all attacks to receive an additional attack at the highest attack bonus.
4th level option of a feat or "Storm of Blows" ability- (requires Rain of Blows) As part of a Full Round Attack, the Kensai may take a –6 penalty to all attacks to receive two additional attacks at the highest attack bonus.

FEATS:
Lightning Fists (Prerequisite: Monk level 4th+, Dex 15+): You can make two extra attacks in a round. All attacks made this round suffer a -5 attack penalty. This feat requires the full attack action.

...of course, the spell Haste, or an item that grants it, is also a great addition.
Anything else?

And assuming you were a Monk who primarily fought unarmed, Weapon Focus: Unarmed would help negate some of the penalties. And the first level of Kensai grants you +1 to hit and to damage with your favored weapon.

Thri-kreen Monk/Master Thrower. Can't recall exactly the choices, but rapid shot, flurry, the multiweapon fighting tree, and the Palm Throw ability, all add up to something like 34 attacks a round, though I believe I'm missing a couple elements.

Edit: With Shuriken, the only monk ranged weapon you can Flurry with.

Find templates/races that add in natural attacks.
and use Rapid shot with Quickdraw as well.

They all stack.

This means that a thrikreen monk with a template to get a tail attack, a weapon with the speed enchantment, and the right feats can do:

1 main hand attack with a monk weapon
...that fluries
and off hand attacks with a non-monk weapon (get the full improved multi-weapon feats)
...throw one of the off hand weapons
...use quickdraw and rapid shot to get a 2nd attack with that off hand weapon.
use haste/speed weapon to get a single extra attack of his choice
and make a tail attack as a secondary natural attack.

Note, the penalties often stack up.

The XXXX/Master thrower is nice, but gets expensive, quickly, if you want even +1 weapons.... 'course, add in Returning, and each one is more expensive, but you only need one per attack, instead of bandoleers of daggers/shuriken/javelin.

fighter with a spiked chain... machine gun tripper build?

thri-kreen using the old 3.0 tempest class? (4 on and off hand attacks, or 4/4/4/4!)

I'm fairly certain this has been done before on the Character Optimization (http://boards1.wizards.com/forumdisplay.php?f=339) board. Asking there should give you more information than you can imagine.

That said: hasted monk Choaker with 'Girillon's blessing' ;)

Add TWF to this monk and other TWF feats.

In 3.5 Monks can use TWf and Flurry together even unarmed.

1) TWF (lowers TWF penalties): Using TWf gives an extra attack so this helps.
2) Improved TWF: Now you get 2 extra attacks more.
3) Greater TWF: 3 extra attacks instead of 2 or 1.

Just thought these deserve mentions.

Technically, palm throw doesn't increase the number of attacks. You're throwing two weapons with one attack roll. As K-Man pointed out, it gets expensive. Additionally, you're getting gypted out of your str bonus to damage.

For an ECL 20 character, don't waste feats on the 2WF/MWF chain, and don't worry about having a high dex. Go with a non-psionic thri-kreen with the multiheaded template, Rapid Shot, Brutal Throw, and a high strength: 17 attacks in a round with no penalties to attack and full strength damage, plus two secondary bite attacks. With boots of speed that's 20 attacks in a round.

It's suprisingly easy to make a character with 32+ attacks.
Also you could just take the thri-keen and give it another pair of arms :D
not only do these builds give a lot of attacks but they deal more damage than a 21 level wizard. i got one in the range of 38-256 damage with an average of 6 criticals (not included). this was made wit ordinary 1d8 weapons with a str mod of 0 and no spells, abilities, other eq and so on.
bab of 19 and all attacks get a +8 bonus so +27 attack roll with a standard action.
no idea what i wrote here. Probably makes no sense but its kinda hard while looking Zatoichi :D

Also the best way to get huge ammounts of attacks is to create a centipede that has 50 pairs of arms wielding +5 Dancing daggers and carrying enough to fully equip 4 of it's kind. I think some already know what this means :evillaugh:

I'm surprised no one's mentioned the dervish's A Thousand Cuts yet.

A thousand cuts (now it's been mentioned!), gorilla arms (from Magic of Incarnum), and also Umbrial Diciple adds reach to you attacks so you could get that and combat reflexes for good AoOs

If you want crazy attacks per round there's something that will beat the **** out of all of these things - It's called a 12 headed Pyrohydra =)

If you can find the Dungeon magazine that focued on Githyanki, there were some feats that increase your number of attacks of opportunity. If you took:

Agile Riposte: When someone misses you, you can hit them with an attack of opportunity.
Double Hit: When using Two-Weapon Fighting, you can make an attack with your second weapon as part of an attack of opportunity.
Karmic Strike: When someone hits you, you can hit them back with an attack of opportunity.
Improved Combat Reflexes: Take 2 attacks of opportunity instead of one (second at -5).
Greater Combat Reflexes: Take 3 attacks of opportunity instead of 1 (third at -10).

You're going to need a high Dex, but since you are a monk that shouldn't be too difficult to focus on anyway. The more times you are attacked, the more attacks you'll get. The best part is that it won't matter if they hit or miss you, you'll be taking 3 extra attacks against them. You could also take Sidestep to improve your maneuverability on the battlefield since you will be able to take several 5-foot steps each round. If you use the Sparring Dumming of the Master (Arms and Equipment Guide page 137), you'll be taking 10-foot steps all over the place.
