I am sorry if this has already been answered.

Let's say I am a fighter (or even better a rogue with Sneak Attack) and a enemy and a friend are grappled.

I read somewhere (Rules of the Game: All about Grappling) that ranged weapons have a 50% of hitting either opponent (assuming sucessful attack role). It doesn't mention hand-held weapons. Am I to assume that a rogue could just walk up and choose the strike (with sneak attack damage) the enemy and leave the friend unharmed?

I have had GM's use either the 50% rule or even the damage goes 50% to each grappler.

I've always used the same 50% miss chance for melee attacks as well as ranged.

This isn't useful, but my group has not been able to figure out the answer for this.

The 50/50 ONLY applies to ranged attacks. See the Combat Modifiers section of your PHB/SRD.

On the "Armor Class Modifiers (http://www.d20srd.org/srd/combat/combatModifiers.htm)" table there is note for the 'ranged' column of the 'Grappling' row that reads "Roll randomly to see which grappling combatant you strike."
