In the DMG it states that if your flight maneuverability is average or below you can not fly backwards.

My question:
if you bull rush a flying creature backwards (that’s maneuverability while flying is average or lower), does it fall to the ground since it can not fly backwards?

Also:
If you grapple a flying creature what kind of check is it to stay flying? And what if you pin them?

I would say that it's knocked backwards for a few feet, then begins to fall on its turn. It can still do the reflex check to recover, though.

In other words .. it stalls ..

I agree ... :P

so would you say for every 10 feet knocked back they fall 10 feet?

and what about the other questions?

No, I'd say that they're knocked back as per usual for the Bull Rush rules, and then fall as per standard falling rules (150ft, DC 20 reflex save to recover or fall another 300ft)

If you Bull Rush a flying creature left, right, forward or backward, the rules don't say it stalls. The rules say it stalls if it fails to maintain its minimum forward airspeed.

Now if you Bull Rush a flying creature, odds are that you'll be in its face, threatening it with a melee attack, when the creature begins its next turn. If the creature must keep flying or stall, odds are that you'll get an attack of opportunity on it when it moves out of a square you threaten. This would be a good opportunity to Trip it, since the Main 3.5e FAQ (page 26) says that tripping a winged flyer makes it stall.
