If I'm prone...

1) Can I use the tumble skill to move elsewhere without standing up first?

2) Is a 5-foot step still free? Is any movement possible?

3) Is any concentration check necessary for spellcasting?

1) Nope. There might be a class ability or feat in some splatbook that'll let you do so, but as far as core rules go, no.

2) Nope. You can't 5-foot step, but you can crawl 5 feet as a move action that provokes attacks of opportunity.
Edit: There's a skill trick in Complete Scoundrel that lets you take a regular 5-foot step while prone with a successful Tumble check. Like all skill tricks, though, you only get to use it once per encounter.

3) Nope. Cast to your heart's content. I suggest the stand spell.

combat acrobat & prone attack are useful feats if your character tends to kiss the floor often.

You can crawl 5 feet as a move action. Crawling incurs attacks of opportunity from any attackers who threaten you at any point of your crawl.But here's the official WotC position on when you can Tumble: As with any movement, you can attempt a Tumble check to avoid any attacks you provoke with this sudden leap. So you can Tumble to avoid AoOs while crawling (moving while prone). Note that you must make an Accelerated Tumbling check for this, because your full move while prone is only 5', and half speed won't get you anywhere.
2) Is a 5-foot step still free? Is any movement possible?To take a 5' step you need to be on your feet, so you'd need to stand up first, then the 5' step would be available. (Note that it's "not an action" rather than a "free action".)
3) Is any concentration check necessary for spellcasting? Not for being prone.

Here's the tumble section in question from the SRD (http://www.d20srd.org/srd/skills/tumble.htm) (emphasis mine):
Tumble at one-half speed as part of normal movement, provoking no attacks of opportunity while doing so. Failure means you provoke attacks of opportunity normally. Check separately for each opponent you move past, in the order in which you pass them (player’s choice of order in case of a tie). Each additional enemy after the first adds +2 to the Tumble DC.
So, I don't know what happens if your five foot crawl is reduced to half speed. Do you still get to move a square? Is there some ruling that states that you always get to move five feet?

Tumbling While Crawling: Though the rules don't say so, there's no reason you can't can (sic) tumble 5 feet as a move action when prone. The check DC is 20.

You try to tumble past or through enemies more quickly than normal. By accepting a -10 penalty on your Tumble checks, you can move at your full speed instead of one-half your speed.

To avoid the AoO you've got to make a Tumble check, and you must include the -10 penalty for Accelerated Tumbling to avoid the AoO while moving the full 5'. The real RAW answer is that the check DC is 25 (actually DC 15 for the basic Tumble check to avoid the AoO, with a -10 Accelerated Tumbling penalty to your roll to move the full 5' rather than half that).

Maybe it's not listed in a book so you can't count it specifically as RAW. It is on the Wizards website, however, so I'd give it a little more weight than a house rule.
