When buying Tomes/Manuals like +5 ones. do you pay the full 137500 cost or the 6,250 gd +25,500 xp cost ?

You pay the market price, just like every item in the book.

Assuming of course there are any for sale.

The price that says market price if your dm allows you to get it. By raw you genrally cant(max gp limit is 100,000gp). You have to find it or make a deal with someone to make it. The 6,250 gd +25,500 xp cost is what you pay to make it.

Well unless you can find a metropolis with a gp limit higher than 100,000gp these items are unavailable for purchase...

Technically speaking, without the aid of scrolls, a 20th level caster probably wouldn't want to make a +5 tome (unless that caster has a way of casting wish/miracle without the xp penalty). Most of the xp costs for the tomes comes from the Wish/Miracle component. Can't craft an item where the xp cost would cause you to lose a level (19,000 xp between level 19 and level 20). Therefore, a 20th level caster pretty much loses a level in creating the +5 tome for some PC. (I thought there was a rule that prevented someone from crafting an item where the xp cost would cause the character to lose a level but I can't seem to find it. If such a rule does exist, that's another reason why a 20th level caster couldn't make it).

Ultimately, check with your DM about such an item. Personally, I don't have +5 tomes floating around for sale in any city and found in some random treasure because I tend to play NPCs as a player would. That is, there better be a damn good reason for me to spend an obscene amount of xp to make a tome for some random PC just for a few gold coins.

Well, an alternate means would be to cast wish 5 times in succession, which might not be feasible for all spellcasters(They might not have high enough int to get a bonus 9th lv slot, or sacrificed one to arcane fire archmage ability..) Thus, the next best thing would be to store them in magic items.

Couldn't you just start with a +1 tome and upgrade it, paying the difference as you would when upgrading a magic weapon, armor, or WI (like an Amulet of Health). That way the XP costs would be spread out.

There is a rule that says you can't lose a level when crafting an item or casting a spell. There is also a rule that lets you save xp for crafting rather than using it to level up.

Well, there are ways to craft +5 tomes without going into Epic levels. It just requires a bit of a work around.

Example: Wizard scribes a few Scrolls of Wish over his career so that he doesn't lose a level. He saves those scrolls down the line to craft the +5 tomes by "storing" the xp hit from the Wish spells in scroll form and using the scrolls instead of casting wish to craft the item. Its possible but requires a lot of work and foresight.

There are set guidelines for determining if an item is an epic item. The +5 tome doesn't fall in that category. I'm just pointing out the difficulty of crafting a +5 Tome and putting it into consideration as to whether it can be purchased somewhere.

When buying them from the market, you pay the 137,500 GP price. When crafting them yourself, you spend the 6,250 GP + 25,500 XP cost.
