If all the PCs are before (or all are after) the monsters, I'll tell them that. It simply makes the combats go a bit faster (no need for them to ask "so who goes next?" all the time).

In or out of character? In character the initiative line-up doesn't exist. Everyone is acting in the same six second span, just some people are a hair faster than others.

Out of character it really depends on the DM, I suppose. I personally don't try to hide it from the players. I don't explicitly show it to them, either, but I make no secret of who's acting next. It's not terribly difficult to figure out after the first round is over, of course. Except for the occasional hold or readied action it doesn't change.
