Okay, are there any practical defenses against a chain-wielding PC w/Improved Trip and the Weapon Focus/Specialization Feats?
Is there some feat or defense option that I've missed?

there are tons of options:

tumble for avoiding AoO's is good.
prone fighting allows you to fight from the ground without penalty and stand up as a free action on a successful hit.
Improved disarm or sunder can shut a chain weilder down fast.
Or use shapechanging magic to get bigger or get more legs or both.
there's also the theif acrobat prestige class which gets a ability to stand up more easily, but I forget the details.
There's also the kip up option to gain your feet as a free action: takes a dc 35 tumble check IIRC.

It may not be of use to you right now, but you could play a dwarf. That +4 racial bonus to avoid being tripped & bullrushed negates Improved Trip nicely.

Stand up for a free action that doesn't provoke. It's fantastic. :D

- Sunder the weapon.
- If you can't get close enough, ready an action to sunder the weapon.
- Use larger creatures (even if they are still tripped, they can reach him while prone).
- Use spellcasters
- Use smokesticks, obscuring mist etc.
- Hide around a corner
- Use flyers
