I was wondering, what's the best way to have this work within the rules?

It's not the same as drowning, unless someone choking you starts the fort saves on round 1. (Plus STR mod to DC?)

Just being in grapple and doing grapple damage?

People fall unconcious first before death, so this could be used as a temporary knockout right?

Would this also work to keep someone silent?

On an unrelated note, what would the DC be for saving against uncounciousness when affected by chloroform? (More a d20 modern problem I guess)

I'd say you'd have to pin the guy, and if you can maintain it for a number of rounds equal to your opponent's Con score (which is how long you can hold your breath), he'd be KO'd.

Anything that bypasses the HP system shouldn't be too easy to implement. =\

Thanks. The rules I was looking for are already in the book. I just didn't notice them.
