The rules here confuses me a bit.

It says if the defender has some concealment, he get a 20 or 50 percent chance that the attack misses. That's ok. But what if the concealed defender want to attack? Does he has the same chance of missing, due to concealment? After the rules I would say: "Yes, of course". But aren't there situations where this don't fit. Let me take an example. You are an elf, standing in the leavs in the top of a tree. You have a clear sight to the ground, but the defender on the ground do not have clear sight of you because of the leaves. Here I would say it's naturally to give the elf 20 percent concealment if the the guy on the the ground attacks, but not vica versa. Are the rules with me or not?

Are there anyone you cant help me out with this?

There are many times when you have concealment but your opponent does not.

Classic examples of this are spells, such as Blur.

The one you mentioned however does not seem to qualify. Generally, if you have concealement because of intervening things, then so do they. I might have the elf get a bonus to hiding, but only because he was up in the sky, not because of the concealment at all.

Remember that people are assumed to be moving , so your elf can not keep the people in view all the time, sometimes they move so that leaves etc. happen to totally block the elve's view.

Instead, you might try something like this:

Standing behind a bead curtain with just your head showing grants you 20% concealement, but they get none.
