Tactics can be all that is needed to win almost any battle. In this case, so long as the creature breathes, you can kill/disable it.

What it takes:
Dust of coughing and Choking ("cursed item") - 2000 gold
+1 Scythe - @2400 gold
Finding a Red Dragon in a cave with tubes - Difficult at times
Fighter with weapon focus scythe, 18 strength, and power attack - Free if made right

Killing a Red Dragon up to Ancient in one hit? - Pryceless :D

The dust if thrown poofs in a 20' radius cloud. If you make the save, you are STILL stunned for 5d4 rounds while choking and coughing. Stunned = Helpless. Helpless = Able to have a coup de grace attack done on you. Scythe = X4 crit multiplier
So, all in all you average 52 damage on a successful critical hit without power attack (add more for power attack). with a fort save of +28 at ancient, the only way they make the DC 62 save is by rolling a natural 20... like I said in the title:

CURSED ITEMS FTW!!!!!

Where does it say that stunned = helpless?

A helpless character is paralyzed, held, bound, sleeping, unconscious, or otherwise completely at an opponent’s mercy. A helpless target is treated as having a Dexterity of 0 (-5 modifier). Melee attacks against a helpless target get a +4 bonus (equivalent to attacking a prone target). Ranged attacks gets no special bonus against helpless targets. Rogues can sneak attack helpless targets.

As a full-round action, an enemy can use a melee weapon to deliver a coup de grace to a helpless foe. An enemy can also use a bow or crossbow, provided he is adjacent to the target. The attacker automatically hits and scores a critical hit. (A rogue also gets her sneak attack damage bonus against a helpless foe when delivering a coup de grace.) If the defender survives, he must make a Fortitude save (DC 10 + damage dealt) or die.

Delivering a coup de grace provokes attacks of opportunity.

Creatures that are immune to critical hits do not take critical damage, nor do they need to make Fortitude saves to avoid being killed by a coup de grace.
A stunned creature drops everything held, can’t take actions, takes a -2 penalty to AC, and loses his Dexterity bonus to AC (if any).
