Does anyone know where you find the guidelines to make and price custom grafts?

Well, I don't know how custom you can get when you still have to take the body part off someone/thing else... but this is my best guess.

Take a look at the craft magical item (http://www.d20srd.org/srd/magicItems/creatingMagicItems.htm#tableEstimatingMa gicItemGoldPieceValues) section in the SRD, add it up according to the effect you want the graft to have, then multiply the price by 2 for the unslotted effect.

Let's make up something... like skin of the lizardman. It should give a natural armor bonus from anywhere from 1 to 5... (A natural armor bonus is the bonus squared × 2,000 gp)

+1 = 2,000 gp
+2 = 8,000 gp
+3 = 18,000 gp
+4 = 32,000 gp
+5 = 50,000 gp

And lets say that you wanted it to grant a +2 or a +4 bonus to swim checks because it still had webbing at the fingers and toes. (A skill bonus (competence) is the bonus squared × 100 gp)

+2 = 400 gp
+4 = 1,600 gp

So lets say as a final product you wanted it to grant a +3 to natural armor and a +2 to swim speed. You'd wind up with...

+3 NA = 18,000 gp
+2 swim = 1.5 x 400 gp = 600 gp
[Because multiple different abilities multiply lower item(s) cost(s) by 1.5]
2 x (18,000 + 600) = 37,200 gp for a custom skin of the lizardman.

If by going with effect, you wind up with something that's too cheap for what it grants, feel free to bump the cost up as you see fit. Of couse the Magic Item Compendium might have a better system than the old DMG one...
