How do you determine if a large creature is affected by the difficult terrain? If the creature takes up 4 squares and the terrain it passes through is only 2 sq' x 1 sq' in width, does it suffer the 2 movement point cost when moving through it?

I couldn't find it in the SRD, or perhaps it is in the books, anyone know?

Unless a creature's description specifies otherwise, or it uses a mode of movement (fly, jump, etc) to avoid it, the creature is affected by difficult terrain, no matter its size compared to the area of difficult terrain. The creature still has to move through the area.

It just seems a little silly when you have a Colossal creature and one 5' square of difficult terrain and it's entire movement is affected by it.

It isn't, only that 5' square is difficult "terrain". It only costs him 10' of movement to cross that one square.

However, presumably we aren't talking about 5' squares since that isn't terrain and wouldn't affect large scale movement. Presumably we are talking about various 5' squares separated such that the creature needs to slow down, avoid tripping or stumbling, etc.

Even a colossal creature needs to put there foot down somewhere. It seems silly that a few one inch square dice on the carpet can keep you from walking across a room at full speed, but once you step on one 4-sider, you honestly have to slow down.
