You were lowered down from the street to the first level and started from there. A ton of Demons, etc... were said to have escaped when the hole was first ripped open? Hellgate Keep (or Ascalhorn as it was once known) sits on the north-eastern edge of the High Forest in the North. Or used to, as members of the Harpers destroyed it in 1368 DR, and the ruins are now guarded by the treant Turlang.
