It was when the Twisted Rune captured him and took him to Selune. His magic in Undermountain went nuts and spewed out monsters all over the place until he was returned to Undermountain.

Undermountain: Stardock has the whole adventure.

I wonder if Wizards might publish endnotes online for books that warrant them, as starwars.com did for The New Essential Chronology (http://www.starwars.com/eu/lit/ref/f20051025/).

Undermountain: Stardock has the whole adventure. To expand a little on what Kuje just said... Undermountain: Stardock was one of the Dungeon Crawl modules for 2E. It was the third and final module set in Undermountain (the other two were The Lost Level and Maddgoth's Castle -- all three are available for free here at WotC:- http://www.wizards.com/default.asp?x=dnd/dnd/downloads)

The Twisted Rune is a cabal of evil spellcasters; they kidnapped Halaster so they could gain his knowledge of portals. With the Mad Mage outside of his playground, the many layers of spells he's wrought down there started going awry. All over the Realms, portals to and from Undermountain were opening. Some grabbed unsuspecting people, pulling them into Undermountain. Other portals dumped monsters into the laps of those unsuspecting people. It was a very rough day in Waterdeep, in particular.

Halaster was eventually rescued by a group of adventurers. He returned to his Underhalls and things returned to normal, more or less.
