The Forgotten Realms started out as just a dungeon with a town to which the PC's could retreat?

No -- where did you hear that? The Realms started as a setting for Ed Greenwood's short stories years before D&D. The World of Greyhawk, on the other hand, did grow around the Greyhawk Castle dungeons and the City of Greyhawk.

Yes, he's sure. And what he said has come from Ed himself, so I tend to take the word of the setting's creator over someone elses on how he created the setting. :)

I can also confirm what Faraer said to be true. I've also read that Ed Greenwood created the Realms himself.

-The Forgotten Realms started out as an inn (Well, that was the first setting ever created there), in Ed Greenwood's first novel about it, "One comes, unhearalded, to Zirta" (or something very similar). That can be found in the Best of the Realms Vol. 2
