I ran a Waterhavian coast patrol back in 2E. It was pretty fun, the guy started out pretty weak with a crappy ship, and with enough pirate takedowns and islands discovered they gained wealth and even got a new ship and decked it out. I had a lot of fun running a aquatic elf fighter npc as well. Sadly the game ended after I tried to give the guys a break from the ship and water thing. I took them on land for a while and the sea campaign died. I made a lot of cool cut out boats to use for minatures though :D

I have played in and run a couple of campaigns, while not totally island hopping, had a fair number of sea adventures in them. One thing I would rekomend is to level them up to around 7th level before actually going majrly off-shore, because the nicer creatures are around that level, and it's harder to survive on the sea than on land (IMO).

Otherwise, there are a fair number of things you could do; here's a few:
Underwater - have an underwater city exploration style adventure, with rings of water breathing. Mad fun.
Pirating - the PC's could be pirates, and have to evade the law, etc. That would work quite nicely.

Well, I could probably think of more, but I'm a bit on overload right now, so I really can't think. Well, you could also incvolve a number of kraken, they're always nice  Also, for more general adive, visit the DM's Advie Thread, in my signature. It's a bit long, but just skim it, ok :)

Sembian privateers, Calshite merchants, Fauerunian explorers along the coast of Chult...

Go crazy. ;)

i ran one once i had a pc as a first mate on a ship who were out hunting pirates they downed the ship took on a few prisoners to bring into port with the ship listing badly to port. This delayed the travel back into port and they were hit by lightning in the middle of a storm crippling the main mast.:omg!: To top it all off during the storm the ship was attacked and pulled into the depths by a kraken.:88e: The pc escaped on one of the life boats along with other members of the crew but more than half along with the capt. was lost in the watery depths. He then traveled the land adventuring and earning enough gold to buy his own ship. needless to say the pc was up in arms wondering when he was going to get his :censored: break.

There are two 2E Forgotten Realms books you should pick up:

The Sea of Fallen Stars
and
Pirates of the Dragon Coast

They have tons of Realms lore about life on and under the sea.
