An idea from the Old Transformers Series:

Bane and Talona cook up a new plan for gaining more power. They create a plague that cause people to hate. It spreads by contact and causes a green glow in the eyes.
(This works best in a world were Bane has Strife, otherwise Cyric will get power as well.)
Think of the Chaos and Trouble it would cause. How would the people in the Realm react to this? What would they do to cure it? How would the Gods react to it?

Just an idea....

For a pretty decent idea like this, check out the movie 28 days later. Obviously, in the realms, if a god caused it, another god will work to reverse it. In this case, you could have the makings of a very large quest. Evil gods cook it up, find a person to start it, and let the vector take over. Heroes and other types begin hearing of, say, genocide in the dalelands, and some head off to investigate. Later, others hear that those same heroes are now abroad, slaughtering indiscriminately. So, smart heroes will find themselves trying to capture a victim, study it, and try to diagnose the problem.

Reaction- most likely, good gods with healing tendencies will figure out the plague is divine in nature, and might send people to recover a powerful healing artifact, tome, or site. No fun at all if this thing can be cured by any cleric. Clerics of Illmater, or a Chosen of Illmater, make for good motivators here.

As for how it would be cured, that's your game.

Yeah I saw that movie and during it I felt that it would work well in a D&D campaign.

Would not all plague's be, hypothetically, divine in nature?
