The PGtF covers the first three, and also allows you to date Underdark specifically to Ukta 16.

Maybe there will be a web-enhancement for Underdark to cover this topic, since I guess it won't be published in any non-related sourcebook. You may try this question in the "Ask the designers" - thread on this board though.
