Alter Self lets you change into any creature of your type up to 5HD and up or down one size category. As a Human able to cast Alter Self, wouldn't I be able to change into the Hybrid Form of a Wereboar? It has 4HD, Humanoid type, and is Large. Also, assuming I can shift into a Wereboar Hybrid Form, what feats do I get from the list given in the Monster Manual stat block?

According to the LGCS:

Only base creatures from the MM are allowable form selections. Creatures with templates, advanced versions of base creatures, and creatures not in the MM are not allowed without campaign documentation. For lycanthropes you cannot change into their hybrid form.

That last part is actually reduntant since Lycanthropes are templeted creatures and thus you couldn't change into one anyway.

Living Greyhawk Alter Self Choices
The following list of choices and their modifiers assumes changes from a base race of human.

Bugbear - +3 natural armor bonus, +4 racial bonus to Hide
Dwarf; deep, hill & mountain – 20 ft. base land speed, Stonecunning , Stability, +2 racial bonus to Appraise and Craft checks involving stone or metal items.
Dwarf, duergar - 20 ft. base land speed, Stonecunning, Stability, +2 racial bonus to Appraise and Craft checks involving stone or metal items, +4 racial bonus to Move Silently, +1 racial bonus to Spot and Listen.
Elf; drow, high, grey, wild, & wood - +2 racial bonus to Spot, Listen and Search. Spot secret doors within 5 ft.
Elf, half-elf - +2 racial bonus to Diplomacy and Gather Information. +1 racial bonus to Spot, Listen and Search.
Elf, aquatic – 40 ft. Swim Speed, +2 racial bonus to Spot, Listen and Search. Spot secret doors within 5 ft.
Gnoll - +1 natural armor bonus
Gnome, forest – 20 ft. base land speed, Small size, +2 racial bonus to Listen and Craft (alchemy) checks, +4 racial bonus to Hide (+8 in forest)
Gnome, rock – 20 ft. base land speed, Small size, +2 racial bonus to Listen and Craft (alchemy) checks

Orc & half-orc – nothing.
Troglodyte - +6 natural armor bonus, Multi-attack, +4 racial bonus to Hide (+8 in rocky or underground areas), 2 Claws (primary) and 1 Bite (secondary) for 1d4 each.

Stonecunning is a racial bonus to Search checks under specific conditions and therefore fulfils the ‘racial bonus to skill checks’ condition of alter self.
The elf racial ability to spot secret doors at 5-ft. is described in the same ability bullet at their racial bonus to Spot and Listen and is an extension of those bonuses and therefore fulfils the ‘racial bonus to skill checks’ condition of alter self.
The current LGCS only allows players to change shape into creatures in the Monster Manual I. Humans are not listed in the MM1 and are thus not technically eligible as a creature LG players may shape change into. Most GMs, however, overlook this technicality.
