Apparently penalties cannot stack if they come from the same source. Does that mean that it cannot come from the same spell?

3.5 PHB glossary:
penalty: A negative modifier to a die roll. Penalties do not usually have a type, and always stack with other penalties (except those from the same source) unless otherwise stated.

For example:

Ray of Emfeeblement does a 1d6+1/level strength penalty. Ray of Exhausting makes your target exhausted, which is a -6 strength and dexterity penalty (fatigued if saved, which is a -2 strength and dexterity penalty).

1. Does this mean that multiple Ray of Enfeeblements on the same target do not stack because the spell description did not specifically say this penalty stacks?

2. Are both of these "strength penalties" from different sources? Or are all spells considered the same "source"?

1. Yes. It means you cannot use ray of enfeeblement on the same target twice.

2. Yes, and No. Yes, these are from different sources. No. All spells are not considered to be "from the same source".
