Based on the new Polymorph subtype spells in PHB 2.

I was noticing that the Troll was 4th level yet CR 5. so CR 4 would be Level 3? I think so. I made it a standard action instead of a swift action like the other ones to cast so it can be cast on others.

5headed-Hydrashape [polymorph subtype]:
Components: V, S
Casting time: standard action
Target: Creature touched
Duration: 1 minute/ level. (D)

Your body begins to expand and split apart as a five snake-like heads explode from your now scaly shoulders. Your form reshapes into that of a terrifying hydra.

Subject becomes a 5 headed Hydra. You gain +20 temporary Hps that disappear at end of the spell.
You become a hydra (keep your own Hps, lose your feats/class features, gain creatures feats, Creature's stats). Use creatures base saves; base Attk bonus. You do not gain the hydra's ability to regrow new heads, but otherwise function as a hydra in all other respects.

I think it is balanced. Bab is at most 5 which is same as warriors should have at level 5 when you gain 3rd level spells. Major benefit is the multiple heads and fast heal (similiar to trollshape but regeneration is stronger).
It is a nice buff, but not overpowered to me.

But I just wanted your input.

Edit: Raven's ideas helped a bit. I won't call it lesser since this is minor Hydraform at most. And I'm not sure if I am goingto make a lesser one (6 heads lol).

I've already created that spell (for the most part). It was largely agreed to be a 7th level spell. See my Hydraform thread (http://boards1.wizards.com/showthread.php?t=633994) for more details. Maybe you could model your "lesser hydraform" after mine.

EDIT: For those who don't want to follow the link...

Hydraform
Transmutation (Polymorph)
Level: Sorcerer/wizard 7
Components: V, S
Casting Time: 1 swift action
Range: Personal
Target: You
Duration: 1 round/level (D)

Your body begins to expand and split apart as a dozen snake-like heads explode from your now scaly shoulders. Your form reshapes into that of a terrifying hydra.

You take on the form and abilities of a 12-headed hydra (MM 157 (http://www.d20srd.org/srd/monsters/hydra.htm)). You gain 60 temporary hit points, which disappear at the end of the spell's duration. You do not gain the hydra's ability to regrow new heads, but otherwise function as a hydra in all other respects. See the description of the new polymorph subschool (PH2 95 (http://www.wizards.com/default.asp?x=dnd/ex/20060501a&page=5)) for more details.

There is a reason why they said seventh level. Maybe 8th, but it is a low powered 8th.
CR=11. Bab +12.

Dude, that is very strong. But good note about not regrowing heads I'll use that.

Mine is only 5 headed. CR 4.

I would say Lesser Hydraform would be about...4th level.

It should probably give +20 temporary hit points and you may want to consider not giving the fast healing 15 (since that is a bit much for a low level spell).

But Trollshape is +30 temp and Troll Regeneration (since you get all the troll abilities) as a 4th level spell.

Lesser is far weaker in that aspect.

Imean, killing a troll form requires fire or acid since it is all subdual.

Hydra is killable. If I change the Hydra form to lower it to 10 Fast Heal maybe? But than it stops being a real polymorph Subtype spell. The basic idea is you just become the creature: no adds or subtracting.

But I'll think about it.
