In your campaign, how are ability score modifiers (Wish, magic items, etc.) handled? Banned, encouraged, more expensive, cheaper, or as the rules say? In my campaign, the price is under the DM's discretion, but otherwise allowed. How about you?

As core rule say, with occasional freebies.

I.E. as a reward I might grant a +1 con instead of a full Wish scroll.

In my campaign, the mechanics are handled the same way they are in tarkin's --> exactly as written in the core rules.

Further, wish can never be purchased by a third party provider. Tomes/Librams, etc. are occasionally found in ancient library ruins, but will not be available for purchase, although commissioning such an item is possible. Of course, at 1 day for every 1000gp value, that often makes for a very long wait!

Um, yeah, I handle them about the same way I do magic weapons and armors. The game is balanced on the assumption that the PCs will have access to these items, but as always, its under complete DM control over when and where the players get them.
