Please PEACH this magic item:

Amulet of Contingent False Life (or Amulet of Revivification, or some other name...suggestions welcome)
Wondrous Item

Created by a wizard of minimum 11th level with Craft Wondrous Item feat.

Single-use item; takes the spot of an amulet/necklace.

Base item at least as valuable as a silver holy symbol (25 gp)
Spells used in creation:
Contingency (6th level arcane, employed at caster level 11; total cost 6*11*50gp=3300gp, plus 1500gp for the focus item)
False Life (2nd level arcane, employed at caster level 10; total cost 2*11*50gp*0.75 similar function discount=750 gp)

Total cost: 25+3300+1500+750=5575gp

When wearer drops to 0 hp or lower (This is the Contingency), the amulet instantaneously casts False Life on the wearer.

Questions:
1) Does this item sound kosher?
2) Have I calculated costs properly?
3) The concept on the "contingency" part is that the wearer doesn't have to personally "command" the item to work, considering he will be unconscious/disabled/dying at the time it is needed...is this unbalanced?
4) Is there any reason the base item couldn't BE a cleric's actual holy symbol?

The purpose in mind for this item is that the party I am DM'ing killed an evil cleric foe that I need to bring back to further a plotline. Interestingly, they killed him on a "lucky shot" just as he was jumping out a 2nd story window to escape them. They did not recover the body, nor did they make any effort to search for it. They did note that he was wearing a holy symbol on a necklace. This NPC certainly would have had the means to own an item priced like the above, and the connections to find a wizard capable of making it for him.

I am considering making this a recurring theme with this foe...that they keep killing him and he keeps finding ways to come back. He could become one of those "recurring villain" types.
