No Base class Caster Levels stack, such as Sorcerer, Cleric or Wizard.

Most Prestige classes that have spellcasting grant you "spells known", "spells per day" and "increased caster level" in a class you have taken previously.

Certain Prestige Classes specify whether it enhances only Arcane or Divine classes, and a few Prestige Classes have their own Caster Level and spell progression that is specifically just for the Prestige Class, such as Assassin.

If a PrC says it grants an increased Caster Level in an Arcane class, then it doesn't matter if you were previously a Sorcerer, a Wizard or a Bard (or various supplemental arcane classes). Same with Divine, if the PrC specifies Divine, ie Cleric, Druid, Ranger/Paladin etc.

Practiced Spellcaster raises your caster level by four, but not beyond your hit dice.

Practiced Spellcaster also applies only to a single class at a time. You can take it more than once and apply it do a different class, if you are a multiclass caster.

AND you can't use it if both of your classes are casters. You basically use your nonspellcasting classes as a bonus to your caster level.

I'm not seeing anything limiting it to spellcaster/non-spellcaster multiclasses. It looks like it should work fine with say, a cleric4/wizard4/mystic theurge X, raising the caster level to character level.

That is wrong. There is nothing limiting it to having a caster class and a non caster class. In fact it specifically gives an example of how the feat works if taken twice with a cleric 4/wizard 5.

The Orange Ioun stone grants +1 caster level. This is independent of any particular level type, so if you were a Sorcerer/Favored Soul/Assassin it would give you +1 caster level for all three classes.
