Just wondering if any one has come across the situation of a player who wanted to craft a lucky 'charm', simpley a magic item of a sort that bestows its benefit as long as the item is carried, and not activated or held.

A luckstone replicates pretty much what you describe:

Stone of Good Luck (Luckstone): This stone is typically a bit of rough polished agate or some similar mineral. Its possessor gains a +1 luck bonus on saving throws, ability checks, and skill checks.

Faint evocation; CL 5th; Craft Wondrous Item, divine favor; Price 20,000 gp.

Precisely what I am trying to get at. What exactly would the limitations be, what would be considered abusive, and what would a DM want to get away with?

The same limitations and potentials for abuse as any other magic item.
Just follow the pricing guidelines in the DMG.
