We are currently playing a level 19 party (all the way from 1st). The wizard has come up with a nasty. He's using a rod of metamagic for empower spell. Empowered Disintegrate. Normally, it would be 38d6 on a failed save, Empowered, it's 57d6. That's an average of 200 points of damage. Heighten that puppy to 9th level, and the save is tough to beat (29 or so). My character is a fighter type, and my save is only 22!

Cast that sucker to interrupt a spellcaster, and the concentration check would be DC 210.

Not bad....
I'm shuddering to think about what the DM is going to throw at us now.

Considering it's an epic spell with empower, I dont think it is that unreasonable..

save of 22 is NOT low... its somewhat average.. you can beat the save 65-70% of the time.... It is nothing if you make the save.. so its fair.. btw empower is calculated by 0.5 * (your dice effects) not with extra dices..

Try empowered meteor swarm.. 24d6 + .5 (damage) damage no save with area effect..

concentration DC 210?? sure, IF you can "hit" the guy that is.. and you have to ready action...

200 damage is nothing at high levels since clerics have heal and all and there would be LOTS of potions and spell effects anyway..

I have no idea how you decided it was an epic level spell; he is using a metamagic rod.

A save DC of 29 vs a save of +22? First, both of those are low for 19th level characters. Second, that means you succeed more often than you fail.

A DC 210 concentration check is, of course, not going to be passed. This is after a touch attack and a failed saving throw, and they can tell that you are readying an action and do something to stop that (such as readying an action of their own).

The lack of a starting Int of 18 and a tome of +5 are not uncommon on non-minmaxed campains. And remember that Spell focus and Greater Spell focus are not that good now. Base Fort Save +22 is quite high, i don't know where you get your numbers, but those are quite good for their level.
