Is there a good way to determine which squares are affected by a flying character who casts a spell with a cone of effect?

If both the attacker and the target are on the ground, it's easy to figure out which targets are affected and which ones aren't. But when you've got a caster flying 20 or 30 feet off the ground, casting cone spells at targets on the ground at some crazy angle, it gets difficult.

How do you do it? Do you just fudge it, and say, "Eh, those two guys get it, but that guy doesn't." Or do you have a more rules-based way to do it?

The most important things to keep in mind are...
1 - the maximum edge of the cone (range)
2 - the diameter of the "flat" part of the cone is usually equal to the range.

Then I suggest looking at it from the side view as well as top-down. You can tell what vertical squares are affected (same method as horizontal; that's the beauty of a cone)
