It's the Warforged card that opens up the fighter, paladin and artificier subsitution levels. Now my question is do I have to make a new character to activate these levels as they come in the Races of Eberron book at levels 1, 2 and 4 or do they activate at different levels in the Mark of Heroes campaign? |

From what I've heard you can no longer re-tool classes at all (unlike LGR, right?) so, as every character which has played a module is already locked into levels 1-4, you'd have to create a new character if you wanted to take one or more of those racial sub. levels. But I might be wrong |

I'm not familiar with the specific expansion card you talk about, but if it is an expansion card you should be able to put it in your stack to unlock the applicable options. If it is a creation card you would need to make a new character, since you need those at creation. |
