Given a creature with 4 arms and a +6 bab.

Can they wield two, two-handed weapons (one on each side?)

If yes, does each weapon do 1.5x str bonus?

Also if yes, do they get 2 attacks with each weapon (because of their bab?)

er.. hmm, well, 'off hand' weaposn usually only get .5 instead, bt you have two off hand.. i'd simply give them full strength bonus instead as a comprimise, your primary weapon woudl still get 1.5 strength.

only if you have improved multiweapon fighting. You'd use multiweapon fighting instead of two weapon fighting because of having 4 arms. and you'd need Greater Multiweapon Fighting to get a third attack with the off-hand weapon.

I seem to recall reading about four-armed sahuagin, where it said that they could only use one two-handed weapon and two light weapons.

There is a section in sword and fist that discusses multi-armed creatures and it specifically states that they can use multiple two-handed weapons but it doesn't cover the logistics of it, hence my post.

Well, if I were the DM I would allow the character to use two two-handed weapons, but at a higher penalty to hit. Since a two-handed weapon is more unwieldy than a regular weapon, and a regular weapon goes from -2(light) to -4(med) I would set it at -6, which is discouraging, but not overplayed. Besides, I would allow the strength and a half per hit. But, I admit, I wouldn't even let a character dream of using both two-handed weapons unless he had "multi-weapon fighting" as a feat.

Easy.

Okay, first, the creature would indeed gain its 1.5 times Str bonus for holding the weapons in two of their hands.

The character would need to take improved Multi-weapon fighting to benefit from the extra attacks with their off hand. In addition, the multi-weapon fighting feat negates off-hand penalties.
