When advancing monsters past 20 HD, do you use epic level rules to determine BAB and base saves? i.e. epic attack bonuses and epic save bonuses? if not, how are those determined? thanks!

No.

Requirements for a monster to use epic AB and saves:
- At least 21 HD.
- At least 1 class level.

How?
- Count racial HD first. Determine attack bonus and number of attacks.
- Count class levels and apply their saves and attack bonuses
-> if the class level is applied when you had 19 or less total hit dice before, it does not use epic attack bonus or saves.
-> if you had 20 or more total hit dice before, use epic attack bonus and saves for the class level.

what about more than 20 HD and no class levels? I know BAB is HD times .5,.75,or 1, but whats the formula for saves?

Good saves increase by 0.5 per HD (starting at 2 for the first HD), while Poor saves increase by 0.33 per HD (starting at 0 for the first HD).

thanks much! edit: do you have a reference for that so I can look it up when I forget? (and I will!)

No real reference other than the table in the front of the PHB.

Btw, I made one little mistake above; the starting points should be 2.5 and 0.33, respectively.
