I think that undead only gain d12s for racial hit die, or if it's granted by a template.
so ghouls who take monk levels get d8s for those levels.
am I right?

Several undead templates specifically state that all current and future hit dice are rerolled as d12s. I am of the opinion that all undead are supposed to work like that, not just the ones that state it.

If the undead was created by a template, all HDs convert to d12s. If an undead without a template takes levels in a class (such as the mummy-lord in the MM who has levels of cleric) add the base HD for the class. (The mummy-lord's cleric levels only give him d8 HD).

This is correct. Only those templates that specifically state that all Hit Dice become d12s do so; all other undead use the Hit Dice for their class level.
