I'm a little confused about the aquatic subtype and the differences between fresh and salt water environments. There seems to be no differentiation between these environments anywhere in the MM whatsoever. So are all aquatic creatures assumed to be able to survive in both fresh and salt water environments, are there fresh and saltwater versions of each aquatic creature, or something else?

Aquatic Creatures are usually fine in both salt and fresh unless stated in the monster's description.

The Sahuagin for example has this ability:

From the SRD:
Freshwater Sensitivity (Ex): A sahuagin fully immersed in fresh water must succeed on a DC 15 Fortitude save or become fatigued. Even on a success, it must repeat the save attempt every 10 minutes it remains immersed.

Gotcha.

It's funny that I only recently realised that most of the creatures that I assumed were amphibious (scrag, merrow etc) are in fact aquatic and have a hard time when on land. Suddenly I'm wondering just how these creatures get into the exitless subteranian pools that seem to appear in many modules...

Many modules don't make sense ecologically. You can find rats or goblins behind locked doors that have not been opened in centuries, with nothing to eat in the room (no den or quarters either), and no other exit.

It's a heroic-fantasy version of Pasteur's spontaneous generation theory. Monsters just are created out of thin air when the environment is propitious.
